Requests
To access the Requests page, click the Service Center tab followed by the Requests navigation link. Your financial institution can set up any number of online request forms that display on the left side of the Requests page, allowing you to complete and submit forms for various banking services. The Requests page also includes interface links on the right side of the page to access the interfaces your financial institution has enabled.
Overview
Numerous online request forms/functions may display on the right side of the Requests page, but the following list includes standard requests that may be available to you. Be aware you only see the links for which your financial institution has set up as a static link; therefore, you may not see all links in the following list:

Deposit Account Request – Used to request a new deposit account as described in the Entering a Deposit Account Request section. Your financial institution will review the deposit account request and contact you.

New ATM/Debit Card – Used to request a new ATM card or a new ATM/Debit card for one of your deposit accounts (i.e., a checking or savings account) as described in the Requesting a New ATM/Debit Card section. Your financial institution will review the completed online request form and issue the requested card(s) if appropriate.

Wire Transfer Request – Used to originate a wire transfer request (refer to the Entering a Wire Transfer Request section). This is a request to initiate a wire transfer and not an actual wire transfer order. Your financial institution will contact you for further information about this request.

Cash Order – Used to enter a cash order (refer to the Entering a Cash Order section). Your financial institution will review the request, debit the appropriate account, and fulfill the cash order at the specified branch.

Check Reorder – Used to order checks as described in the Requesting a Check Reorder section. Your financial institution will follow standard check ordering procedures to handle the requested order.

Loan Account Request – Used to request a new loan account (refer to the Entering a Loan Account Request section). Your financial institution will review the loan account request and contact you.

Safe Deposit – Used to apply for one or more safe deposit boxes at your financial institution as described in the Requesting Safe Deposit Box(es) section. Your financial institution will follow standard procedures to fulfill the order.

Address Change – Used to submit an address change (a new, corrected, or alternate address) to your financial institution as described in the Entering an Address Change section.

One Time Transfer to External FI – Used to enter a one-time request for a funds transfer to another financial institution (refer to the Requesting a One Time Transfer to a Financial Institution section).
Contact your financial institution if you have questions about the online request forms or request functions available to you.

Click the Deposit Account Request link on the Requests page.

When the Account Type Request Ownership page displays:
-
Select either the Joint Ownership Account or the Single Ownership Account option.

Click Continue to go to the next step or Cancel to return to the Requests page.

If you selected the Joint Ownership Account option in step 2:
-
On the Joint Ownership Deposit Account Request page,
-
Select at least one or more Account Types and any number of Electronic Banking Services.

Type or select the appropriate data in the Applicant Information section for the primary applicant. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

On the Joint Ownership Deposit Account Request – Co-Applicant page,
-
Type or select the appropriate data in the Applicant Information section for the co-applicant. All fields marked with a red asterisk are required.

Click Continue to go to the next step, Back to return to the Joint Ownership Deposit Account Request page described in step 3a to review/change the data, or Cancel to return to the Requests page.

When the Joint Deposit Account Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the Account Type Request Ownership page described in step 2 to review/change the data, or Cancel to return to the Requests page.

If you selected the Single Ownership Account option in step 2:

On the Single Ownership Deposit Account Request page,
-
Select at least one or more Account Types and any number of Electronic Banking Services.

Type or select the appropriate data in the Applicant Information section for the applicant. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

When the next Single Deposit Account Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the Account Type Request Ownership page described in step 2 to review/change the data, or Cancel to return to the Requests page.

Click the New ATM/Debit Card link on the Requests page.

On the New ATM/Debit Card Request page:
-
Select either the New ATM Card or the New ATM Card/Debit Card option.
-
You may enter the Reason why you are requesting the new card (or two cards for a joint account).

In the Account Number field, type the number of your checking or savings account where transactions will be posted for the new card(s).
-
Type or select your name, address, phone number, and ID information. All fields marked with a red asterisk are required.
-
To request two cards for a joint account:
-
Select the This is a Joint Account and two cards are required box.
-
Type name information for the second card. The First Name and Last Name are required.
-

Click Continue to go to the next step or Cancel to return to the Requests page.

When the New ATM/Debit Card Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the page described in step 2 to review/change the data, or Cancel to return to the Requests page.

This is a request to initiate a wire transfer and not an actual wire transfer order. Refer to the detailed note at the bottom of the Wire Transfer Request Information page.

Click the Wire Transfer Request link on the Requests page.

On the Wire Transfer Request Information page:
-
Type or select the appropriate Sender Information, Transfer Amount and Date, Receiving Financial Institution Information, Beneficiary Information, and any Intermediary Financial Institution Information. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

When the Wire Transfer Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the page described in step 2 to review/change the data, or Cancel to return to the Requests page.

Click the Cash Order link on the Requests page.
-
On the Cash Order page:
-
Type your name or the name of your business in the Customer/Business field and the Branch Location for pickup at your financial institution where you want to pick up the cash followed by the Pickup Date and the Pickup Time (in the format HH:MM AM or PM).

Type the Count or the number of Bills and/or Rolls of Coins that you want to pick up in the cash order. As each number is entered, the appropriate amounts and totals are automatically incremented.

Click Continue to go to the next step or Cancel to return to the Requests page.

When the Cash Order Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the page described in step 2 to review/change the data, or Cancel to return to the Requests page.

Click the Check Reorder link on the Requests page.

On the Order Checks page:
-
Type the number of your checking account that requires new checks in the Account Number field and the Starting Check # for the order.

As an option, you may also select the Number of Boxes of checks that you want.

In the remaining fields on the page, type or select your name, address, phone number, and ID information. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

When the Order Checks Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the page described in step 2 to review/change the data, or Cancel to return to the Requests page.

Click the Loan Account Request link on the Requests page.

When the Loan Account Request page displays:

Select either the Joint Ownership or the Single Ownership option.

Click Continue to go to the next step or Cancel to return to the Requests page.

If you selected the Joint Ownership option in step 2:

On the Joint Ownership Loan Account Request page,
-
Select the appropriate Account Type (Secured, Unsecured, or Line of Credit) and type an optional Product name.

Type or select the name, ID, address, and phone information for the primary applicant. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

On the next Joint Ownership Loan Account Request page,
-
Type or select the name, ID, address, and phone information for the co-applicant. All fields marked with a red asterisk are required.

Click Continue to go to the next step, Back to return to the Joint Ownership Loan Account Request page described in step 3a to review/change the data, or Cancel to return to the Requests page.

When the Loan Account Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the Joint Deposit Account Request page described in step 3a to review/change the data, or Cancel to return to the Requests page.

If you selected the Single Ownership Account option in step 2:

On the Single Ownership Loan Account Request page,

Select the appropriate Account Type (Secured, Unsecured, or Line of Credit) and type an optional Product name.

Type or select the name, ID, address, and phone information for the applicant. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

When the next Loan Account Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the page described in step 4a to review/change the data, or Cancel to return to the Requests page.

Click the Safe Deposit link on the Requests page.

When the Account Type Request Ownership page displays:

Select either the Joint Ownership Account or the Single Ownership Account option.

Click Continue to go to the next step or Cancel to return to the Requests page.

If you selected the Joint Ownership Account option in step 2:

On the Joint Safe Deposit Request page,
-
Type the Branch Location of your financial institution where you want the safe deposit box(es).
-
In the Number of Boxes field, retain the default number 1 or type a multiple number of boxes.

Type or select the name, ID, address, and phone information for the primary applicant. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

On the Joint Ownership Safe Deposit Request – Co-Applicant page,

Type or select the name, ID, address, and phone information for the co-applicant. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

When the Joint Ownership Safe Deposit Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the Joint Safe Deposit Request page described in step 3a to review/change the data, or Cancel to return to the Requests page.

If you selected the Single Ownership Account option in step 2:

On the Single Safe Deposit Request page,

Type the Branch Location of your financial institution where you want the safe deposit box(es).

In the Number of Boxes field, retain the default number 1 or type a multiple number of boxes.

Type or select your name, ID, address, and phone information. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

When the Single Ownership Safe Deposit Request page displays asking you to confirm the information listed on the page, click Confirm to confirm the data, Edit to return to the Single Safe Deposit Request page described in step 4a to review/change the data, or Cancel to return to the Requests page.

Click the Address Change link on the Requests page.

On the Change of Address page:
-
Indicate if this is a New Address, Correction, or Alternate Address.
-
In the top section, type your name information exactly as it appears on statements followed by a valid e-mail address. All fields marked with a red asterisk are required.

In the Old Address section, type or select your current address information exactly as it appears on statements.
-
In the New Address section, type your new address information.
-
In the Additional Information section:
-
Select All Accounts or the type of accounts to which the address change should be applied in the Select Accounts to Change field.
-
Indicate if you want to Change your Internet Banking Profile. That is, change the address for your online banking profile.
-
If you want the new address to apply to every bill, notice, letter, and statement your financial institution sends to you, select Yes in response to the question Do you wish to use the new address for all communications including statements?

Type any Comments for the address change.

Click Continue to go to the next step or Cancel to return to the Requests page.

When the Change of Address Request page displays asking you to confirm the address change information listed on the page, click Confirm to confirm the data, Edit to return to the page described in step 2 to review/change the data, or Cancel to return to the Requests page.

Click the One Time Transfer to External FI link on the Requests page.

On the Add One Time External Transfer page:

Type or select all of the information requested in both the Transfer Information and the Destination Information section. All fields marked with a red asterisk are required.

Click Continue to go to the next step or Cancel to return to the Requests page.

When the One Time External Transfer Request page displays asking you to confirm the transfer information listed on the page, click Confirm to confirm the data, Edit to return to the page described in step 2 to review/change the data, or Cancel to return to the Requests page.
